CDC FAX INFORMATION SERVICE INTERNATIONAL TRAVEL DIRECTORY
The following documents are available from the CDC FAX INFORMATION SERVICE. These documents are updated as necessary. To receive a document call 404-332-4565 and follow the prompts. To enter document numbers, choose option 1, and when requested, enter the six digit document number and then the # key. Then, listen to the prompts to enter additional documents or enter your FAX telephone number. Your FAX telephone number should include your Area Code plus telephone number (10 digits) and then the # key. You may enter up to five documents. If you do not receive your Fax in a reasonable amount of time, check to see if your fax machine is on and available for transmission. The Fax system will make 5 attempts to deliver the document. After each attempt the FAX system waits 5 minutes, then inserts the request at the bottom of the queue. The number of pages is given after the title "- 2" = 2 pagesDocument Directories
