  Dear Colleagues:

        On behalf of the National Association of County Health Officials, I
 am pleased to provide you with the National Directory of Local Health
 Departments for 1991.

        The National Directory was developed for two main purposes: to
 provide a means of contacting any and all local hearth departments in a
 specific area; and, to respond to the requests from NACHO constituents for
 a comprehensive up-to-date listing of public health at the local level for
 interorganizational communication. I hope that this National Directory, one
 of NACHO's efforts to make the role of local health departments more
 visible, will be a useful resource.

       Information gathered through NACHO's National Profile of Local Health
 Departments project was the basis for this National Directory. We have gone
 to considerable effort to update our information, and would like to thank
 the many health officials who have helped us with this ongoing activity.
 NACHO would very much appreciate your providing complete or corrected
 information for the next edition of the National Directory.

       I hope that you find this National Directory to be a valuable resource.

INTRODUCTION

      The National Association of County Health Officials is dedicated to
 improving the capacity of local health departments to assess local health
 needs, develop public health policies, and ensure delivery of needed
 community services.  Communication among federal, state, and local health
 agencies and other health organizations is critical to achieve this
 improved capacity. We believe that this National Directory will enhance that
 communication and we are pleased to provide it for your information and use.

      The initial listing of local health departments was taken from the
 National Profile of Local Health Departments, NACHO 1990.  The initial list
 has been updated using supplementary sources as available including the
 NACHO sustaining member mailing list and current local listings requested
 from State-Local Liaison Offices.

      As discussed in the National Profile, there is great variability in
 local health department structure nationally.  Since a main purpose of this
 National Directory is to provide comprehensive information on local entities,
 a local health department is defined here, as it was in the National Profile,
 as:

    an administrative or service unit of local or state government which
    is concerned with public health, carries responsibility for the health
    of a jurisdiction, and is smaller than the state.

 The following entities are included in this definition:

 * local health departments as defined by the Association of State and
   Territorial Health Officials (ASTHO), 1983*;

 * agencies meeting the ASTHO definition but having fewer than one full-time
   employee;

 * local branches of state health departments which provide core public
   health services;

 * independently operating nursing and environmental units such as those in
   Colorado;

 * district health departments whose jurisdictions are comprised of other
   local health departments and provide assistance to independent local
   health units.

 Subunits or satellite offices of local health departments and non-
 governmental agencies (such as those in Alaska, which, for the most part,
 provide local health services through nonprofit corporations) are not
 included. Therefore, the National Directory includes 3169 entries, of which
 2945 are local health departments fulfilling a variety of roles and meeting
 varying state statutes, and 119 are district health departments. Also
 included are 105 county boards of health in Iowa.

      The entries appear by state and are in alphabetical order by local
 health department name. For states with district health departments, the
 listing of these follows the local health department listing. Each state is
 introduced by a map showing the types and locations of the local health
 departments.

 * An officiai (govemmental) public health agency which is, in whole or in
   part, responsible to a substate governmental entity or entities. An entity
   may be a city, county, city-county, federation of counties, borough,
   township, or any other type of substate govemmentai entity. A local
   health department must: "have a staff of one or more full-time professional
   public health empioyees (e.g., public health nurse, sanitarian); deliver
   public health services; serve a definable geographic area; have
   identifiable expenditures and/or budget in the political subdivision(s) it
   serves."

 database used for this National Directory which does build on the Local
 Health Department Directory by USCLHO in March 1988.
