CC37.NMES87.PUF5.DOC CC37.NMES87.PUF5.FMT ABSTRACT 1987 National Medical Expenditure Survey: Public Use Tape 5 Institutional Population Component, Facility Questionnaire Supplement Nursing and Personal Care Homes and Facilities for the Mentally Retarded This documentation describes one in a series of public use tapes from the 1987 National Medical Expenditure Survey (NMES), which provides a new and extensive set of data on the use of health services and on health care expenditures in the United States. NMES continues a series of national health care expenditure surveys, most recently the 1980 National Medical Care Utilization and Expenditures Survey (NMCUES) and the 1977 National Medical Care Expenditures Survey (NMCES). Like the earlier surveys, NMES used a national probability sample of the civilian, noninstitutionalized population in a household survey. In view of the increasing volume and cost of long-term and institutional care, NMES included as well the population resident in or admitted to nursing homes and facilities for the mentally retarded over the course of the survey year. To provide more focused estimates of the provision of long- term care rendered in the community, the household survey oversampled population segments known to be at risk of needing or using services for chronic impairments or long-term illness, in particular the elderly and the functionally impaired. Because of continuing policy concern with populations with restricted access to the health care delivery system, poor and low-income families and the black and Hispanic minorities were oversampled as well. In addition, the American Indian and Alaska Native population living on or near reservations and eligible for the Indian Health Service was included in NMES on the basis of a separate household sample. Together, the major components of the National Medical Expenditure Survey provide national estimates of health status and estimates of insurance coverage and the use of services, expenditures, and sources of payment for the period from January 1 to December 31, 1987. The reports of health care expenditures and insurance coverage obtained in the household surveys are supplemented by additional surveys; most important among these are the Health Insurance Plan Survey of employers and insurers of consenting household survey respondents, and the Medical Provider Survey of physicians, osteopaths, and inpatient and outpatient facilities, including home health care agencies, which were reported as providing services to any member of the noninstitutionalized population sample. A Medicare Records component is used to provide a record check on 1987 eligibility status and claims information of all Medicare beneficiaries, including those in the institutional population. The survey of persons who were resident in or admitted to long-term care facilities (nursing and personal care homes and facilities for the mentally retarded) at any time in 1987 provides data on their health care use and expenditures for the entire survey year, both within the institution and in the community. This information, as well as extensive data on health and functional status, demographic characteristics, and living arrangements and insurance coverage at the time of admission, was obtained from two primary sources. The surveys in institutions (SII) collected data from facility administrators and designated staff; they were complemented by surveys of next-of-kin (SNK) or other knowledgeable persons in the community to obtain additional personal history and information on insurance coverage and health services use and expenditures both within and outside the institution. Taken in conjunction, these surveys will yield comprehensive, population-based estimates that permit separate analyses of most groups of policy interest, including those presently outside of the scope of the various public and private financing mechanisms. The NMES panel design included several rounds of household interviews and followed institutional residents over a full calendar year to assess the effects of changes in health status, income, employment, and eligibility for public and private insurance coverage on both the use of services and on public and private expenditures for care. The 1987 National Medical Expenditure Survey was sponsored by the National Center for Health Services Research and Health Care Technology Assessment, now the Agency for Health Care Policy and Research. The Indian Health Service cosponsored the Survey of American Indians and Alaska Natives. The Health Care Financing Administration provided consultation and technical assistance during the development of the survey instruments. The data were collected and are being edited and published in accordance with the confidentiality provisions of the Public Health Service Act and the Privacy Act. A series of further public use tapes is in preparation to ensure timely access to these data by the research and policy community. Field work for the institutional population component and the household survey was conducted by Westat, Inc., Rockville, Maryland, as the primary contractor and by NORC, University of Chicago. Field work for the Survey of American Indians and Alaska Natives was conducted by the Council of Energy Resource Tribes and Stephen R. Brand Associates under subcontract to Westat. Data processing for the public use files is being provided by Social and Scientific Systems, Inc., Bethesda, Maryland. TECHNICAL AND PROGRAMMING INFORMATION General Information This documentation describes one in a series of public use tapes from the 1987 National Medical Expenditure Survey (NMES). The tape contains data collected with a facility questionnaire supple- ment (FQS) in phase 3 of the NMES Institutional Population Compo- nent (IPC). Released as SAS and EBCDIC data sets, this public use tape provides additional information on a nationally representative sample of nursing and personal care homes and facilities for the mentally retarded. The data are being released prior to final data cleaning and editing in order to provide prompt access to NMES by the research and policy community. Further data from the IPC and from other NMES components will be released on the same basis. For a listing of previously released public use tapes from NMES, see Attachment 3. The following documentation offers a brief overview of the type and level of data provided, the content and structure of the files, and the codebooks, and programming information. Attachment 1 also contains the instrument used to collect the information in this data file. A previous NMES public use file contains data for the facilities responding to the facility questionnaire in phase 1. A revised version of the weight variable for these facility data is currently being released as an update file. All facility level public use data sets can be linked using the facility identification number variable (OFIDX). Data File Contents and Structure This public use data set contains a data file with one record for each sampled nursing home and facility for the mentally retarded responding to the facility questionnaire supplement (FQS) administered during phase 3 (early 1988) of the Survey in Institutions. Initial facility questionnaires were administered at all primary sampled facilities during phase 1 (early 1987) and collected facility data such as certification status, services provided, size, and staff characteristics. They also were used to determine eligibility of the facility for the IPC. The facility questionnaire supplement was administered to all primary sampled facilities which participated in phase 3 of the IPC in order to supplement these facility data. It included questions on special Alzheimer's units in nursing homes, including the capacity of current units and plans for future expansion. The FQS also included some questions for nursing homes which had been asked previously only of facilities for the mentally retarded. The codebook provided for this data file contains the following information: Unique facility identifier Survey administration variables Sequential variables based on the facility questionnaire supplement in the order in which the items appear in the questionnaire Weight and variance estimation variables. Most variable labels are abbreviated versions of the questionnaire items and include the question number or location in the instrument (e.g., front cover). A complete variable listing in alphabetical order and by codebook position is provided for cross-reference. A copy of the facility questionnaire supplement is included in Attachment 1 (Exhibit 34) to this public use tape to permit a full understanding of the content and wording of each item, the structure of questionnaire sections, skip patterns and administrative information. Omissions, Edits and Imputations The data file contains the majority of data collected in the facility questionnaire supplement. A few variables were excluded to protect the confidentiality of facility data. For these an explanatory note is provided at the end of the codebook. Box items for use by the interviewer in following skip patterns, interview times, interviewer and respondent names, and similar items were likewise omitted. Data edits were largely restricted to correcting deviations from skip patterns and recoding text fields to preserve confidentially. More extensive editing was done for the set of variables corresponding to questions related to current or planned Alzheimer's Disease programs or units (questions 10-19). This included reconciliation of inconsistent responses for related items, editing of outlier values, and the use of weighted sequential hot decking for imputation of missing values (V.G. Iannachione. 1982. Proceedings, Seventh Annual SAS Users International Conference:759). For all variables with extensive edits including imputations, both the edited and unedited versions are provided. Variable Naming and Coding Conventions The codebook contains variable information and frequency distributions for a total of 1,391 facility records, comprising 759 nursing homes and 632 facilities for the mentally retarded. For each variable, separate unweighted frequencies are provided for each of the two types of records and for the combined group of 1,391 facility records. The codebook describes a SAS data set and provides the following programming identifiers for each variable: IDENTIFIER DESCRIPTION __________ ___________ VAR NAME SAS variable name (maximum of 8 characters) DESCRIPTION SAS variable label (maximum of 40 characters) FMT NAME SAS name used in the SAS FORMAT program statements on this tape which define and label the categories by which the variable can be tabulated TYPE Type of data: numeric (indicated by NUM) or character (indicated by CHAR) LEN Number of bytes per variable, as stored in SAS format VAR NUM Order of the variable within the record NOTES Indicator of an explanatory note(s) corresponding to the variable In general, variable names reflect the content of the variable, with an 8 character limitation. For edited versions of original variables, the edited variable name is identical to the original variable with an "X" appended (and truncated when necessary to comply with the 8 character limitation). For variables corresponding to specific questionnaire items, the question number is included in the variable label. The following reserved code values are used: VALUE DEFINITION _____ __________ - 1 INAPPLICABLE Question was not asked due to skip pattern within the questionnaire. - 7 REFUSED Question was asked and respondent refused to answer the question. - 8 DK Question was asked and respondent did not know the answer. - 9 NOT ASCERTAIN Interviewer did not record the data. -999 NO SRCE DATA Used for constructed variables to indicate that original source data had at least one missing value (i.e., -7, -8, or -9). Sample Design, Sampling Weights and Variance Estimation The IPC sample was selected according to a stratified three- stage probability design, with facility selection in the first two stages. The 1986 Inventory of Long-Term Care Places served as the facility sampling frame. The final stage of selection consisted of a sample of residents as of January 1, 1987, and a sample of admissions between January 1 and December 31, 1987. The sample was designed to yield unbiased national and regional estimates of health care parameters at the facility and resident level. At both levels, the information is classified as to the type of institution; that is, nursing and personal care homes and facilities for the mentally retarded (Cohen, S.B., P. Flyer and D.E.B. Potter. 1987. Sample Design of the National Medical Expenditure Survey, Institutional Population Component; available from AHCPR). The sampling weights developed for the IPC facility data reflect the different probabilities of being selected into the IPC sample. These result from the complex multi-stage sample design of the survey. The facility weights were also adjusted for facility level nonresponse to the phase 1 facility questionnaires and further poststratified using data from the Inventory of Long-Term Care Places on the following facility-level measures: facility type, cost strata, region, certification, status, ownership and bed size. Additional adjustments were made for nonresponse to the phase 3 facility questionnaire supplement and to adjust for duplication in the sampling frame. More detailed information can be found in a preliminary report on the IPC sample design (Flyer, P. 1990. Preliminary Report: Sample Design for the Institutional Population Component; available from AHCPR). Accurate estimates at the facility level for the U.S. population of nursing and personal care homes and facilities for the mentally retarded require the use of weighted data. To facilitate the derivation of national population estimates for IPC data, a facility questionnaire supplement weight, WFQS, is provided. Unweighted counts for applicable variables in the IPC facility questionnaire supplement are provided in the codebook for an evaluation of the number of cases in the various categories. Weighted and unweighted counts and distributions for selected variables are provided as Attachment 2.B to this public use tape. Variance estimates of sample statistics require that the complex nature of the IPC survey design be taken into account for hypothesis testing and for the construction of confidence intervals. Data from complex survey designs require special consideration with respect to variance estimation and analysis, due to the departure from simple random sampling assumptions. Several variance estimation techniques are currently available that will approximate the sampling variance for statistics derived from the NMES Institutional Population Component. Three generally accepted and frequently used techniques for obtaining variance estimates for sample statistics from complex surveys are the Taylor Series linearization method, Balanced Repeated Replication, and the Jackknife Method (Cox, B.G., and S.B. Cohen. 1985. Methodological Issues for Health Care Surveys. Marcel Dekker, Inc.). Due to user facility and computational efficiency, the Taylor series linearization method of variance estimation is recommended for use with IPC data. The technique is compatible with a multi-stage, stratified sampling design with at least two primary sampling units drawn from each stratum. The requirement of paired sampling units was not initially satisfied by the IPC sample design, due to a number of certainty selections of large, IPC-eligible institutions. To allow for variance computations, these primary sampling units were paired as members of pseudo strata, linking facilities with similar characteristics based on the facility measures used for stratification in the IPC sample design (facility type, cost strata, region, certification status, ownership, bed-size, state, and zip code). This strategy will generally result in conservative estimates of the sampling variance. To obtain variance estimates of sample statistics by means of standard statistical package programs, two additional variables must be used in concert with the appropriate sampling weight: STRATFQS, which indicates the respective pseudo stratum, and PSUFQS, which denotes the primary sampling unit within a given pseudo stratum. Appendix 2C contains a listing of several well-known variance estimation programs that are appropriate for use with complex data. Programming Information The files on this public use tape are contained on an unlabelled, 9 track, 6250 bpi tape. The SAS User's Guide: Basics (5th edition) provides information on installation of a SAS Transport file for various operating systems. File 2 contains the technical documentation stored as an Operating System (OS) EBCDIC file containing ASA carriage control characters in the first byte in each record, which will direct the line printer to skip lines, begin a new page, etc. This technical documentation can be copied to disk and retrieved on-line to view and/or modify using a text editor program such as WYLBUR. File 3 is an EBCDIC file containing the SAS program format statements used to generate the frequencies as shown in the codebook. FCAREPRC To protect the identity of the facilities, a continuous variable was created indicating the percentage of residents where Medicare was a payer for facility charges. Facilities with an unknown value for number of total residents or number of Medicare residents have a value of -999. All source data used to construct this percentage are as reported in the FQS. FCAREPRC was not reconciled to other IPC public use data. FRETPRC To protect the identity of the facilities, a continuous variable was created indicating the percent of mentally retarded residents in the facility. Facilities with an unknown value for number of total residents or number of mentally retarded residents have a value of -999. All source data used to construct this percentage as reported in the FQS. FRETPRC was not reconciled to other IPC NMES data items. FSACOTH The "other, specify" text field was excluded from this public use file because of the large amount of confidential (state specific) data in the field. FSADEMXX These variables were edited and missing values FSADEVX were imputed. PRESCHE To protect the identity of facilities, a continuous variable was constructed indicating the percent change from the number of residents reported in the phase 1 facility questionnaire to the number reported in the phase 3 facility questionnaire supplement. Facilities with an unknown value for total number of residents at the time of FQS administration have a value of -999. PSUFQS These variables must be used to obtain variance SRATFQS estimates of sample statistics by means of standard statistical programs using the Taylor series linearization method. They denote the strata and the primary sampling unit (PSU) within given strata. WFQS The facility level weight adjusted for nonresponse. Unbiased estimates at the facility level for the U.S. population of nursing and personal care homes and facilities for the mentally retarded require the use of weighted data.
