DSN: CC37.NNHS8586.NOK.UPDT ABSTRACT Note The original version of the Public Use Data Tape Documentation of the Next-of-Kin Component of the 1985 National Nursing Home Survey was released in 1990. When a problem was discovered, the data tape and its accompanying documentation were immediately recalled. Changes have been made to the data tape and the documentation revised. Thus, data will differ between the original 1990 NOK release and the current release. Anyone having purchased the original NOK data tape items should contact NCHS in order to receive the 1991 version of the Public Use Data Tape Documentation of the Next-of-Kin Component of the 1985 National Nursing Home Survey. Use of NOK Data With the goal of mutual benefit, NCHS reguests the cooperation of recipients of data tapes in certain actions related to their use: A. Any published material derived from the data should acknowledge the National Center for Health Statistics (NCHS) as the original source. It should also include a disclaimer which credits any analyses, interpretations, or conclusions reached to the author (recipient of the tape) and not to NCHS, which is responsible only for the initial data. B. Consumers who wish to publish a technical description of the data will make a reasonable effort to insure that the description is not inconsistent with that published by NCHS. This does not mean, however, that NCHS will review such descriptions. C. Authors should notify NCHS of published articles which utilize the NOK data tapes and provide a reprint to: Ilene Gottfried Division of Analysis, Room 1080 National Center for Health Statistics 6525 Belcrest Road Hyattsville, Maryland 20782 Data Purchase and Use Agreement The Public Health Service Act (42 U.S.C. 242m(d) provides that the data collected by the National Center for Health Statistics (NCHS) may be used only for the purpose for which they were obtained; any effort to determine the identity of any reported cases, or to use the information for any purpose other than for health statistical reporting and analysis, would be against the law. NCHS does all it can to assure that the identity of data subjects cannot be disclosed through public-use data sets; all direct identifiers, as well as any characteristics that might lead to identification, are omitted from the data set. Nevertheless, it may be possible in rare instances, through complex analysis and with outside information on sample cases, to ascertain from the data set the identity of particular persons or establishments. Considerable harm could ensue if this were done. Therefore, the undersigned gives the following assurances with respect to all NCHS public-use data sets: - No one will use the data in these sets in any way except for statistical reporting and analysis; - No one will release the data sets or any part of them to any person who is not a member of this organization, except with the approval of NCHS; - No person having access to the data sets will attempt to use them to learn the identity of any person or establishment included in any set; and -If the identity of any person or establishent should be discovered inadvertently, then (a) no use will be made of this knowledge, (b) the Director of NCHS will be advised of the incident, (c) the information that would identify an individual or establishment will be safeguarded or destroyed, as reguested by NCHS, and (d) no one else will be informed of the discovered identity. General Information This material Provides documentation for users of the Micro-Data Tape of the NextofKin (NOK) Component of the 1985 National Nursing Home Survey conducted by the National Center for Health Statistics. Section IV "Description of the NOK" includes information on the history of the 1985 National Nursing Home Survey, the sample, and data collection procedures. References are Provided in Section V. The NOK Questionnaire is Presented in Section VI. Section VII provides technical details concerning the tape (number of tracks, record length, etc.). Section VIII provides a detailed description of the contents of each data record, by location.METHODOLOGY Introduction The 1985 National Nursing Home Survey (NNHS) is a nationwide sample survey of nursing and related care homes, their residents, their discharged patients and their staff conducted by the National Center for Health Statistics from August 1985 to January 1986. Descriptive material on data collection, field procedures, and questionnaire development for the entire NNHS including a detailed description of the sample design, estimation procedures and qualifications of the data has been published elsewhere (1). Statistics on nursing homes, their current residents, discharges and registered nurses and a variety of other topics have also been published (1-8). This document focuses on the Next-of-Kin (NOK) component of the 1985 NNHS. The NOK supplements the information collected during the facility contact on the current and discharged resident samples. It was designed to collect information not readily available from patient records or other sources in the nursing home on the factors affecting Patterns of nursing home and health care facility utilization. Information was obtained on the resident's status prior to the sample admission, past use of nursing homes, hospital admissions and discharges, activities of daily living at admission, reasons for entering the nursing home, method of payment, current vital status, and living arrangements. The NOK component was conducted by the National Center for Health Statistics in collaboration with the National Institute on Aging. Sample Design The scope of the 1985 NNHS included all types of nursing homes. The sample of 1,220 homes was selected from a sampling frame of 20,479 nursing and related-care homes. The frame consisted of all homes in the 1982 National Master Facility Inventory (NMFI), homes identified in the 1982 Complement Survey of the NMFI as missing from the 1982 NMFI, facilities that opened for business between 1982 and June 1984, and hospital-based nursing homes obtained from the Health Care Financing Administration. The second-stage sampling of current and discharged residents was carried out by the interviewers at the time of their visits to the facilities. The samples were selected in accordance with Ispecific instructions to assure a probability sample. The sampling frame for current residents consisted of all residents on the register of the facility on the evening prior to the day of the survey. Current residents who were physically absent from the facility due to overnight leave or a hospital visit but who had a bed maintained for them at the facility were included in the sampling frame. A sample of five or fewer current residents per facility was selected. The response rate for current residents was 97 percent. The sampling frame for discharges was the total number of persons discharged alive or dead during the 12 months prior to the survey date. Persons who were discharged more than once during the 12 month prior to the survey and then readmitted to the sample nursing home were also eligible to be included in the discharged resident sampling frame. A sample of six of fewer discharges was 95 percent. All residents with a completed Current Resident Questionaire (CQR) or a Discharged Resident Questionaire (DRQ) were eligible for the NOK. The current resident file contained 5,243 cases and the discharged resident file contained 6,023 sample discharge cases (see Public Use Data Tape Documentation, National Nursing Home Survey, 1985). Since the DRQ sample is an event sample, an individual resident could have more than one stay in the discharged resident sample and/or could have stays in both the current resident and discharged resident samples. The NOK, however, was designed to follow residents and not events. Thus, only the first stay for any resident was eligible for the NOK. Eightyone residents had more than one Sampled stay. Forty-three of these involved discharged residents who later became current residents. Two of the 43 had two Sample discharged prior to the stay selected for the current resident Sample. Thirty-eight of the 81 residents had more than one case on the discharge resident file and no cases on the current resident file. One of these 38 had three additional stays. Deleting the 85 additiona1 stays for these 8l residents left 11,181 people eligible for the NOK interview. Data Collection Procedures Six separate questionnaires were used in the 1985 NNHS to collect a variety of information about long-term care facilities and their residents. Although the NOK questionnaire was one of these six instruments, it was administered by using a Computer-Assisted Telephone Interview (CATI), and is not a part of the facility contact. The other five components in the study, which were conducted in the facility, included: (I) The Facility Questionnaire a Personal interview with the facility administrator to obtain basic information about the physicaj structure of the facility, its Certification status, its Size, the number of residents Served, services Provided, 0Wnership, and Staffing; (2) The Expense Questionnaire - a self-administered questionnaire distributed to the administrator, accountant or bookkeeper to obtain data on the facility's operating expenses, i.e. payroll, food, rent, etc; (3) The Nursing Staff Questionnaire - a self administered questionnaire which gathered information on the work experience, hours, activities, education, training, salary, and opinions about recruitment and retention issues of RNs working in nursing homes; (4) The Current Resident Questionnaire (CRQ) - an interviewer completed questionnaire administered to a designated facility staff person who consulted the resident's medical record to obtain demographic data and information on health status, activities of daily living, hospital and nursing home admissions history, and sources of payment for subjects in the facility the night prior to the facility visit; and (5) The Discharge Resident Questionnaire (DRQ) - conducted in the same manner as the CRQ, but for those residents who had been discharged dead or alive from the facility during the 365-day period preceding the day of the facility visit. Unlike the other components of the 1985 NNHS, the Next-of-Kin component was conducted using a Computer-Assisted Telephone Interview (CATI) system. Preparations for the NOK component began in the summer of 1985 with the programming of the computer software. The training of the supervisors and interviewers was held the first week of October 1985. Interviewing was conducted from October 1985 through October 1986. Eighteen cases from the NNHS pretest which are included in the NOK file were conducted in 1984. Information on potential respondents for this component was collected as part of the CRQ and DRQ interviews. Attempts were made to identify respondents who could provide information about the resident's condition prior to being admitted to the nursing home. Respondents to the DRQ/CRQ were asked to provide information on the names and addresses of the resident's next-of- kin, of facilities where the resident had been discharged if applicable, and of relatives and friends of the resident. As the DRQs and CRQs were returned from the field, all information on up to five potential respondents was examined, extracted, and keyed and later transferred to the CATI data file. For each CATI case, a respondent roster displayed the name, address, telephone number, and relationship to the subject of each person listed. Each CRQ and DRQ was reviewed to identify the first respondent, usually the next-of-kin, to be contacted. However, if the subject had been discharged alive into the community the subject was always contacted first. Other potential respondents were then displayed in priority order. If no other respondents were available, an institutional representative from another health facility to which the resident had been discharged was designated as the respondent. For some cases, the nursing home where the subject was residing at the time of facility contact was identified as the respondent. When a facility respondent was contacted information was collected only on the subject's activities after the date of the facility contact. The questions asked of facility respondents are so noted on both the next-of- kin questionnaire and next-of-kin codebook. CRQs and DRQs for which sufficient information was not available for telephone interviewing were assigned a code that identified them as nonactive CATI cases. The reason the case was nonactive was also specified. Each potential respondent was assigned a code to indicate the type of respondent to be contacted. This code was used by the CATI to fill in proper Pronouns and verbs and also determined which set of questions would be asked. Respondents were identified as subjects, proxies or facilities. The nature of the information collected in the NOK also depends on the status of the resident at the time of the facility contact. Thus, residents were divided into four groups - those who were current residents at the time of field data collection; those who had been discharged and were known to be deceased; those who had been discharged and who were not known to be deceased; and those who had more than one record on the CRQ/DRQ files (overlap cases). Reference data from the CRQs/DRQs were also programmed into the CATS system. The data included the subject's ID, the name of the sample facility, the subject's name, sex, and date of birth, the date of the field interview, Sample DRQ/CRQ admission and discharge dates, final status of Subject at the CRQ/DRQ field interview, and the ID's of other records in the CRQ and DRQ file for that subject (overlap ID's). The person designated as primary respondent was mailed an introductory letter, signed by the Director, NCHS, approximately one week before being contacted by the telephone interviewer. Primary respondents for whom no mailing address was available were read an introduction at the beginning of the interview which described the contents of the letter. The primary respondent for each case was always called first. If attempts to reach that person failed, interviewers then attempted to call the other potential respondents. When a person was contacted, that person was asked if he/she was able to answer questions about the subject If not, the person was asked for the name, address, and telephone number of someone who could respond This "new" respondent became part of the respondent roster, and the interviewer attempted to contact that person. NOff interviews were completed for 9,077 subject5 At the time of contact, 3,026 subjects were found to be deceased. While editing the NOK data file, discrepancies were discovered concerning the sex Variable. The discrepancies have been investigated and the correct data appears on the NOK tape. As a result, four subjects have a different sex code on the NOK tape than they had on the CRQ/DRQ tape. For subjects with NOK data, please use sex information from NOK rather than from the CRQ/DRQ data files. Estimation Procedures The information contained on the micro-data tape pertains to a subset of the original current resident and discharged resident samples. Data from the NOK for the two resident samples are combined on this micro-data tape. However, the current resident and discharged resident samples were designed to be analyzed separately. While it is possible to combine the samples in certain instances, the procedures for doing so are not discussed here. Weighting procedures for the NOK will take into account the objectives of the analysis, design of the current resident and discharged resident samples, nonresponse to the NOK and the structure of the NOK file. In addition, some analyses may require more complicated weighting strategies than are discussed here. A statistician should be consulted in those instances. It is essential that the user understand how the NOK file was constructed. Recall that in order to be included on the NOK file sufficient information on the name and telephone number of a suitable respondent (a "next-of-kin") had to have been collected on the CRQ/DRQ and the NOK interview had to have been Successfully completed. In addition, since the original discharged resident sample was an event sample, it is possible for an individual to appear in the sample more than once. It is also possible for an individual included in the CRQ sample to also be included in the DRQ sample. The NOK file contains only one record for these cases and the referent stay is the first Sampled Stay. For example, if an individual has two sampled stays on the discharged resident sample with discharge dates of 4-10-84 and 10-22-84 and was also selected into the current resident Sample, the NOK record for this individual refers to the earliest discharge (4-10-84) as the sample stay but then collects information on the other stays in the section of the questionnaire that collects information on nursing home utilization subsequent to the sample stay (Question 24). The NOK collects information on the characteristics of the resident at the time of his/her admission to the sampled stay as well as information on nursing home utilization prior and subsequent to that stay. The weighting procedures used to analyze this data will depend on the objectives of the analysis. For example, information on utilization history can be used to supplement the information available on the resident questionnaires. In this case, the data on the NOX tape would be matched to the CRQ/DRQ tapes and non-response to the NOK would be treated as missing data. The weights provided on the CRQ and DRQ data tapes are also included on the NOK tape in columns 388-395 and 412-419, respectively. These weights may be used for making estimates of the subset of CRQ/DRQ cases responding to the NOK questionnaire. These weights do not include adjustments for nonresponse to the NOK. Alternatively, the data available on the NOK may be central to the analysis and the investigator might want to use weights that take nonresponse to the NOK into account. Nonresponse to the NOK questionnaire can result from failure to obtain sufficient information on the CRQ/DRQ to contact a "next-of-kin" or failure to obtain an interview given that name and phone number of a respondent was obtained. Two weights for each sample case (current resident or discharged resident) are provided for this purpose. One of these estimators weights the NOK data to the number of CRQ or DRQ cases with an identified "next-of-kin" respondent. This estimator is the product of the baseweight for sample current or discharged residents, adjustments for the presence of "next-of-kin", and adjustments for nonresponse to the NOK questionnaire given a respondent was identified. The second estimator weights the NOK data to the total number of DRQ (or CRQ) cases. This estimator assumes that persons without a "next- of-kin" have essentially the same characteristics as those for whom "next-of-kin" were listed. This estimator is the product of the CRQ/DRQ baseweight and adjustments for nonresponse to the NOK questionnaire. The use of the weights provided are complicated by the design features of the CRQ, DRQ and NOK. Recall that the DRQ is an event sample. That is, events, rather than persons are sampled. Thus, an individual can appear more than once on the sampling list. While the CRQ is a sample of persons and not events, some current residents were also included in the discharge sample if they were discharged from the nursing home in the last 12 months. The NOK component, while using the basic CRQ and DRQ samples, follows persons. This is a particular problem for the DRQ sample. All information for persons sampled more than once is included on one record with the reference date for the utilization history questions (Question 24) being the earliest sampled discharge date. Thus, information on the characteristics of the person at the time of admission (Questions 2-19) is collected only for this first sampled admission. If an investigator wants to make inferences about discharge "events" it is necessary to add cases to the file which duplicate the information for those persons who fall in the discharge sample more than once. Information which identifies the ID numbers of each discharged resident's other sampled stays is found in positions 53-59, and 60-66. When records are duplicated to add cases to represent all discharged resident stays, the ID numbers in Positions 1-7 of the added records should be changed to match he ID numbers identified in positions 53-59, and 60-66 of the original record. For example, a resident has two stays on the discharge file and the first sampled discharge has an ID number of 1000001 (positions 1-7). The ID number of the second stay (1000002), is stored in positions 53-59. When the information is duplicated, the ID for the duplicate stay should be changed to 1000002. In addition, stay information must be rearranged to identify the appropriate sampled stay and stay sequences for each new record. Furthermore, information on subject characteristics prior to admission for other than the first sampled stay will be missing. Similarly, if the investigator wishes to make inferences about all current residents, it is necessary to add current resident cases to the NOK file for each resident with stays in both the discharge and current resident samples by duplicating the NOK information for the discharge resident case and changing the status to "current resident" by changing the ID number. For example, cases with ID numbers in positions 67-73 should be duplicated and treated as CRQ cases with the ID number changed to the CRQ ID found in positions 67-73. Stay information must also be arranged to identify the appropriate sampled stay and stay sequences for each new record. Moreover, because all of these cases were originally sampled as current resident cases (but retained as discharge cases on the NOK file), the CRQ weights in positions 388-395, 396-403, and 404-411 should be used to make estimates for current residents. The DRQ weight fields could be blanked out for these resident cases. The 32 cases that were included in both the CRQ and DRQ samples are the only cases on he NOK file with both CRQ and DRQ weights. Analyses which make inferences about Persons must take into account the fact that the probability of being selected for the survey increases with the number of nursing home stays during the sample Period, i.e, persons with many stays are more likely to be Selected into the samples. There are two ways to do this. One way is to restrict estimates to cases where the sampled stay is the first eligible stay that the individual had during the sampling frame. Or, the investigator can adjust the baseweight by dividing by the Probability of an individual (not events) being Selected and adjusting for nonresPonse. The calculation of the probability of selection for an individual with multiple stays is complex and should be computed under the guidance of a statistician. The information needed to do both of these modifications is available in the CRQ and DRQ files. Reliability of Estimates As in any Sample Survey, the results are subject to both Sampling and non-sampling errors. Non-sampling errors include errors due to response bias, questionnaire and item response, recording and Processing errors. To the extent possible, the latter types of errors were kept to a minimum by methods built into Survey Procedures such as interviewer training, observation of interviewers, and other quality checks. Because survey results are subject to both Sampling and non-sampling errors, the total error is larger than errors due to sampling variability alone. The Standard error is primarily a measure of the variability that occurs by chance because only a sample, rather than the entire universe, is surveyed, The standard error also reflects part of the measurement error, but it does not measure any systematic biases in the data. It is inversely proportional to the square root of the number of observations in the sample. Thus, as the sample size increases, the standard error generally decreases. The chances are about 68 out of 100 that an estimate from the sample differs from the value that would be obtained from a complete census by less than the Standard error. The chances are about 95 out of 100 that the difference is less than twice the standard error and about 99 out of 100 that it is less than 2 1/2 times as large. The standard errors used for this survey were approximated using the balanced repeated-replication procedure. This method yields estimates of overall variability through observation of variability among random Subsamples of the total sample. A description of the development and evaluation of the rePlication technique for error estimation has been published (9-10). To derive error estimates that would be applicable to a wide variety of statistics and could be prepared at moderate cost, Several approximations were required. Rather than calculate standard errors for Particular estimates Sx, the calculated variances for a wide variety of estimates were fitted into curves using the empirically determined relationship between the Size of an estimate X and its relative variance (relvarX). This relationship is expressed as: 2 S b relvarX= -2 =a+ - X X Where a and b are regression estimates determined by an iterative procedure. The relative standard error is then derived by taking the square root of the relative variance curve. Figure I shows relative Standard error estimates for estimated number of residents, and estimated number of discharge5 based on cases found on the NOK file. These two curves can be used for estimates of residents and discharges, regardless of estimator. The relative Standard error (RSE(x)) of an estimate X may be read directly from the curves in Figure I or, alternatively, may be calculated by the formula: RSE(X)= square root +a+b/X+ where the appropriate constants a and b for the estimate X are defined in Table I. Thus, for example, the relative Standard error for estimates of current residents on the NOK file may be calculated by the formula: RSE(x)= square root +-.000l56 + (469.9227/x)+ where X is the estimated number of current residents of interest. Discharged residents .000549 522.6861 In NCHS reports, estimates that have a relative standard error of 30 percent or more are considered "unreliable" and are indicated with an asterisk. Because of the relationship between the relative standard error and the estimate, the standard error of an estimate can be obtained by multiplying the estimate by its relative standard error. Thus, for example, in Figure I, an estimate of 5,200 current residents has a relative standard error of 30 percent. Its standard error is 0.30 X 5,200 = 1,560. To approximate the relative standard error (RSE (p)) and the standard error (SE(p)) of a percent p, the following equations are used: RSE(p) = square root + b(100-p)/py+ SE(p) = p(RSE(p)) where p = (x/y) 100 = the estimated Percent. The approximation of the relative Standard error or the standard error of a percent is valid when at least one of the following conditions is Satisfied: (a) the relative standard error of the denominator is 5 percent or less (11) or, (b) the relative standard errors of the numerator and the denominator are both 10 percent or less (12).
