BACKGROUND
     Approximately seven million persons in the United States are known to
 have diabetes, and an additional five million may unknowingly have the
 disease. Each year more than 650,000 new cases of diabetes are identified.
 In terms of human suffering, persons with diabetes face not only a shortened
 life span but also an increased probability of incurring acute and chronic
 complications.  In 1987, patients with diabetes spent approximately 27
 million days in the hospital. A conservative estimate of the cost in 1987
 attributable to diabetes is $20.4 billion.

     Currently, public health programs at the local, state, and federal levels
 are directed toward reducing the morbidity and mortality from diabetes.
 Health officials need to be able to measure accurately the magnitude of the
 diabetes disease burden in order to establish priorities, develop policy,
 target high-risk groups, plan programs, and evaluate the impact of the
 programs. Public health surveillance, the ongoing and systematic collection,
 analysis, and dissemination of health data, can provide these measurements.

     Until recently, no unified national surveillance system existed for
 monitoring trends in diabetes and its complications.  In the past year, the
 Epidemiology and Statistics Branch of the Division of Diabetes Translation,
 Center for Chronic Disease Prevention and Health Promotion, Centers for
 Disease Control, established an ongoing surveillance system that will
 systematically compile national data on diabetes and its complications. In
 April 1990, the Division of Diabetes Translation published its first
 surveillance report(1), which covered data from 1980 through 1987.  That
 report presents information on the prevalence and incidence of diabetes,
 diabetes mortality, hospitalizations, and several diabetes complications,
 including cardiovascular disease, lower extremity amputations, ketoacidosis,
 and end-stage renal disease.

     For the surveillance report, we relied upon data from two large ongoing
 population-based surveys, the National Health Interview Survey and the
 National Hospital Discharge Survey, to determine the prevalence of diabetes
 and several of its complications.  Although the surveys provide information
 on the prevalence of diabetes and several related conditions in the United
 States, their design limited our analysis to the four major Bureau of the
 Census regions: Northeast, North Central, South, and West. At present,
 state-specific data can be obtained only for mortality and end-stage renal
 disease.

     We recognize the important need for state-specific data to plan and
 evaluate programs. The Division of Diabetes Translation has entered into
 cooperative agreements with three state health agencies to explore methods
 for developing state-based surveillance systems for diabetes.  We anticipate
 that methods and approaches that are identified and formulated in the
 cooperative agreements will be useful for developing similar diabetes
 surveillance systems in other states.

     In the interim, we are providing "synthetic" state-specific estimates of
 the disease burden due to diabetes.  To create these estimates, we have
 applied national rates for a particular condition of interest (e.g.,
 hospitalizations for lower extremity amputations) to the population within a
 state to estimate the number of persons with the condition. These estimates
 were initially generated in response to requests from several coordinators of
 diabetes control programs for information about their respective states.
 Subsequently, we have produced estimates for all the states and the District
 of Columbia. To avoid the pitfalls of over-interpretation, users of these
 data should become familiar with the methods used to generate these
 estimates.

METHODS
     We used the latest available data from various public access data tapes
 to generate a fact sheet for each state. For most of the diabetic conditions,
 we estimated the number of cases by multiplying the national rate for the
 condition times the appropriate state population. For statistics on mortality
 and end-stage renal disease, we used actual counts from vital statistics
 records and Medicare data, respectively.

     We estimated the number of persons known to have diabetes by multiplying
 national diabetes prevalence rates by the 1988 state population. State
 population numbers were obtained from intercensal estimates from the Bureau
 of the Census(2).  These estimates represent the 1988 resident population
 (including military personnel) of the state. Diabetes prevalence rates were
 obtained from the 1987 National Health Interview Survey.  We multiplied the
 age-, race-, and sex-specific rates by the state population within each age,
 race, and sex group, and summed counts over the appropriate groups to obtain
 marginal totals. For example, the number of white males aged 0 to 44 years
 who have diagnosed diabetes was estimated by multiplying the national
 diabetes prevalence rate for this group times the number of white males of
 this age who reside in the state.  To obtain a marginal total, such as the
 number of persons aged 0 to 44 years who are known to have diabetes, we
 summed the number of diabetic persons in this age across all race-sex groups.

     Stable prevalence estimates were available for whites and blacks, but not
 for other race groups. Therefore, we assumed that the rates for these other
 groups were the same as those for blacks, except where noted on the fact
 sheets.

     The annual total mortality due to diabetes was obtained from 1987 state
 vital statistics records. We included deaths in which International
 Classification of Diseases, 9th Revision, (lCD-9) rubric 250 (diabetes) was
 listed on the death certificate as an underlying cause or a contributory
 cause. The diabetes-related deaths are reported by age, race, and sex.

     We used data from the National Hospital Discharge Survey (NHDS) to
 calculate rates of diabetes-related hospitalizations in U.S.
 noninstitutional, nonfederal hospitals(3).  To determine these rates, we
 selected all discharges for which any of the up-to-seven listed discharge
 diagnoses were coded as lCD-9 rubric 250. We used estimates of the total 1987
 population of the United States as the denominators in calculating general
 population rates. These rates were applied to each state population to
 estimate the number of hospital discharges for which diabetes was one of the
 listed diagnoses.

     NHDS data were also used to estimate the number of hospital discharges
 for cardiovascular disease among persons with diabetes. We determined the
 number of hospital discharges in 1987 for which major cardiovascular disease
 (lCD-9 390-448), ischemic heart disease (lCD-9 410- 414) or stroke (lCD-9
 430-434, 436-438) was listed as the primary diagnosis and diabetes (lCD-9
 250) as a secondary diagnosis. Age-specific rates of hospitalization were
 calculated by using, as the denominator, the estimated number of persons in
 the United States with diabetes, as determined from the 1987 National Health
 Interview Survey. These rates were applied to each state's diabetic
 population to estimate the number of hospital discharges.

     In reporting cardiovascular disease mortality among persons with
 diabetes, we used information from 1987 state vital statistics records. We
 enumerated the deaths for which major cardiovascular disease, ischemic heart
 disease, or stroke was listed as an underlying cause of death and diabetes
 was listed as a contributory cause.

     For diabetic ketoacidosis (DKA), hospital discharges were identified from
 the NHDS by using lCD-9 rubric 250.1 (diabetes with ketoacidosis).  National
 hospital discharge rates for this condition were applied to each state's
 population.  For mortality due to DKA, we used 1987 state vital statistics
 records to identify deaths for which lCD-9 250.1 was the underlying cause.

     Data for lower extremity amputation (LEA) were obtained from the NHDS.
 Diabetes-related LEAs were identified by using lCD-9 procedure code 84.1
 (amputation of lower limb) and lCD-9 disease code 250 (diabetes). Traumatic
 amputations of the toe, foot, and leg (lCD-9 disease codes 895-897) were
 excluded from analysis. To estimate the number of LEAs, we multiplied the
 1987 U.S. age-specific rates by state population estimates for each of the
 age strata and summed over all strata.

     To estimate the number of new cases of blindness due to diabetes, we used
 1988 data from the Blindness Registry maintained by the Massachusetts State
 Commission for the Blind(4).  Recent national data on blindness caused by
 diabetes are not available. For each state, we multiplied age-specific rates
 of blindness due to diabetes by the estimated number of persons with diabetes
 in each age stratum.  The total number of cases of blindness was calculated
 by summing over all age strata.

     For end-stage renal disease (ESRD), we obtained 1986 data from the ESRD
 program management and medical information system of the Bureau of Data
 Management and Strategy, Health Care Financing Administration.   This system
 maintains information on about 90% of all persons who undergo treatment for
 ESRD.  The fact sheets present the actual number of patients in a given state
 who in 1986 began ESRD treatment reimbursable through Medicare financing.
