Users should be aware that these changes may account for large numbers of records containing blanks in these variables. In particular, the Family Questionnaire section B on Health Insurance and section F on Family Characteristics contain quite a few questions that were changed during the survey. There were three different versions of some of those questions.o Two aspects of NHANES III, especially, should be taken into account when conducting any analyses: the sample weights and the complex survey design. With the extensive oversampling of blacks, Mexican Americans, young children and older adults in NHANES III, it is IMPERATIVE that sample weights be used in all analyses in order to interpret the findings correctly.o Examine the range and frequency of values of a variable before conducting analyses of data. The range may include unusual or unexpected values. The frequency counts may be useful to determine which analyses may be worthwhile.o Interviewers rated the reliability of responses at the completion of interviews. While all data collected are presented on this data file, it is strongly recommended that responses rated "unreliable" should be omitted from analyses.o The Public Health Service Act (Section 308(d)) provides that the data collected by the National Center for Health Statistics (NCHS), Centers for Disease Control and Prevention, may be used ONLY for the purpose of health statistical reporting and analysis. Any effort to determine the identity of any reported case is prohibited by this law. NCHS does all it can to assure that the identity of data subjects cannot be disclosed. All direct identifiers, as well as any characteristics that might lead to identification, are omitted from the dataset. Any intentional identification or disclosure of a person or establishment violates the assurances of confidentiality given to the providers of the information. Therefore, users will:1. Use the data in this dataset for statistical reporting and analysis only.2. Make no use of the identity of any person or establishment discovered inadvertently and advise the Director, NCHS, of any such discovery.3. Not link this dataset with individually identifiable data from other NCHS or non-NCHS datasets.By using these data, you signify your agreement to comply with the above- stated statutorily based requirements.PREFACEThis Public Use Data File has been edited very carefully. Nevertheless, due especially to the large number of data items, some errors may have gone undetected. Please bring to the attention of NCHS any errors in the data file or the documentation. In publications, please acknowledge NCHS as the original data source. The acknowledgment should include a disclaimer crediting the authors for analyses, interpretations, and conclusions; NCHS should be cited as being responsible for only the collection and processing of the data. In addition, NCHS requests that the acronym NHANES be placed in the abstracts of journal articles and other publications based on data from this survey in order to facilitate the retrieval of such materials through automated bibliographic searches. Please send reprints of journal articles and other publications that include data from this tape to NCHS at the address below.Division of Health Examination Statistics Use of NHANES Data National Center for Health Statistics 6252 Belcrest Road - Room 1000 Hyattsville, Maryland 20782A suggested citation for publications using data from the first three years of NHANES III is: "Source: National Center for Health Statistics, NHANES III, 1988-91". This citation should also appear at the bottom of all tables containing NHANES III data from 1988-91.Public Use Data Files for the third National Health and Nutrition Examination Survey will be released through the National Technical Information Service (NTIS) as soon as the data have been edited, validated, and documented for all six years. The data files will be available for purchase from NTIS by March, 1996. A list of NCHS Public Use Data Tapes that can be purchased from NTIS may be obtained by writing the Data Dissemination Branch, NCHS. A diskette containing an electronic bibliography of journal articles citing data from all of the Health Examination Surveys and National Health and Nutrition Examination Surveys is also available by writing to the Data Dissemination Branch, NCHS. A version of the SETS software package will be available on CD-ROM containing NHANES III data. Information about the CD-ROM/SETS version of NHANES III data can be obtained from the Data Dissemination Branch.Data Dissemination Branch National Center for Health Statistics 6525 Belcrest Road - Room 1018 Hyattsville, Maryland 20782 (301) 436-8500NTIS can be contacted directly at:NTIS-Computer Products Office 5285 Port Royal Road Springfield, Virginia 22161 (703) 487-4807 GENERAL NOTESo Numeric responses: Users should pay careful attention to questions throughout NHANES III forms that are coded using two variables to record a numeric response. The first variable contains the numeric portion of the response and the second variable contains the units of measure. All of these questions were recoded and replaced with standardized variables. For example, responses in days, weeks, or months were recoded to number of times per month.o Rounding: Responses that included fractional answers were rounded off when the code categories were integers only. Fractions of less than 1/2 were dropped; fractions of 1/2 or more were rounded up to the next whole integer. An example would be a response of 2 1/2 years which was rounded up to 3 years. Rounding was not done if units of measure allowed conversion of fractional responses to an integer response. For example, a response of "1 1/2 packs of cigarettes" was converted to 30 cigarettes since the variable allowed for expression of the answer in cigarettes as well as packs.o Ranges: If a response was given as a range or an imprecise number, rules were used to code the response if the interviewer was unable to obtain an exact number. For example, if a person said "in my early 50's", "in my mid- 30's", or "in my late 20's", the response was coded as 53 for "early 50's", 35 for "mid-30's", and 28 for "late 20's". If a range was given, the mid- point of the range was used and any fractions were rounded off. For example, a response of "30-35" was coded as 33.o Which AGE variable to use: Each sample person has an age at screening interview which is almost always calculated using the date of interview and the date of birth. For each examined sample person, there is an age at exam calculated using date of exam and date of birth. Since the sample person was selected into the survey based on age at screening interview and since this determined which components of the survey he/she was eligible to receive, the most appropriate age to use in any NHANES III analyses is "age at interview" (HSAGEI and HSAGEU). There are a few exceptions to this rule. In certain analyses of infants and young children, age at exam may be a more appropriate choice because of the many changes that occur in growth and development during this time period. In general, age at screening interview is the appropriate age variable to use in most NHANES III analyses.o Multiple responses possible: The user should be aware that there are a number of questions asked during the NHANES III interviews where multiple affirmative responses were possible. In these situations, the number of "yes" responses to the question will be more than the number of sample persons who were asked the question.o Sections of questionnaires not included in data file: Some questions or sections of questionnaires are not released on public use data files. These data are the sensitive and personal data that were collected in the survey for the purpose of longitudinal follow-up studies and are not released by NCHS for confidentiality reasons. Some sections of the questionnaires were used for administrative purposes (Diabetes Review in the household youth questionnaire) and are not being released. Other sections (Vitamin, Mineral and Medicine Usage) are still being processed and will be released at a later time.o Changes in questionnaires during six-year survey: Every attempt was made to keep the questions exactly the same during the entire six years of NHANES III. However, because of certain Public Health Service requirements, some additional questions were added at the end of the third year of NHANES III. The primary areas where these changes occurred were in the household family questionnaire but other components changed as well. The family questionnaire changes occurred in the areas of income questions, food security, and health insurance coverage. The NHANES III public release data files have been created to reflect the version of the questionnaires published in "Plan and Operation of the Third National Health and Nutrition Examination Survey, 1988-94", that is, the versions used at the end of the second phase of the survey. For those "new" questions not asked until the last three years of the survey, all the data are blank for those persons interviewed during the first three years.o Unlikely or inconsistent responses or data values: The NHANES III data were edited carefully for skip pattern and range pattern and range errors as well as for consistency between related questions. Somewhat arbitrary but reasonable limits were usually set regarding what extreme values were possible or impossible. If a response was determined to be impossible, that response was edited and recoded "blank but applicable". Food frequency questions were and exception; even if the frequency stated seemed highly improbable, answers were not changed. Analysts are free to exclude extreme values if they wish. Respondents were not always consistent in their answers to questions, either because of misunderstanding or not remembering precisely. Inconsistent responses were examined, but if it could not be determined which was the "correct" response, then both values may have been left as coded.o Other and specify responses: Responses to "other" and "specify" were recoded ("backcoded") to existing to existing categories, if possible. For responses that could not be recoded, new code categories were created if the information was deemed analytically useful. Caution should be used in interpreting data from these new categories because there is no way of knowing which other respondents would have selected one of the new categories if they had been given the options.NHANES III Examination Data File------------------------------------------------------------------------DATASET=EXAM N=15884 DOCUMENTATION DATE=8/24/95 ------------------------------------------------------------------------ General Data ------------------------------------------------------------------------ Positions Item description Source SAS name Counts and code and notes ------------------------------------------------------------------------1-5 Sample person sequence number SEQN 15884 00003-22226NHANES III Examination Data File------------------------------------------------------------------------ Body Measures Component------------------------------------------------------------------------ Positions Item description Source SAS name Counts and code and notes ------------------------------------------------------------------------6-10 Measured height in centimeters BMPHT 14071 073.6-199.8 84 88888 Blank but applicable 1729 Blank11-16 Measured weight in kilograms BMPWT 14033 006.85-206.44 122 888888 Blank but applicable 1729 Blank17-21 Body mass index BMPBMI 13996 10.54-67.31 159 88888 Blank but applicable 1729 BlankNHANES III Examination Data File------------------------------------------------------------------------ Physician's Examination Component------------------------------------------------------------------------ Positions Item description Source SAS name Counts and code and notes ------------------------------------------------------------------------22-24 First systolic blood pressure PEP6GK1 8327 064-244 3782 888 Blank but applicable 3775 Blank25-27 First diastolic blood pressure PEP6GK5 47 000 8281 006-134 3781 888 Blank but applicable 3775 Blank28-30 Second systolic blood pressure PEP6HK1 8315 068-242 3794 888 Blank but applicable 3775 Blank31-33 Second diastolic blood pressure PEP6HK5 22 000 8296 006-126 3791 888 Blank but applicable 3775 Blank34-36 Third systolic blood pressure PEP6IK1 8297 068-238 3812 888 Blank but applicable 3775 Blank37-39 Third diastolic blood pressure PEP6IK5 25 000 8271 010-128 3813 888 Blank but applicable 3775 BlankNHANES III Examination Data File------------------------------------------------------------------------ 24-Hour Dietary Recall Component------------------------------------------------------------------------ Positions Item description Source SAS name Counts and code and notes ------------------------------------------------------------------------40 Recall status code DRPSTAT 14784 1 Reliable and complete 345 2 Reliable, but incomplete 102 3 Unreliable 29 4 Interview lost due to computer malfunction 141 5 Breastfeeding infant or child 229 8 Blank but applicable 254 Blank41-42 Recall day DRPRDAY 1279 01 Sunday 1463 02 Monday 1773 03 Tuesday 2861 04 Wednesday 2638 05 Thursday 3974 06 Friday 1404 07 Saturday 238 88 Blank but applicable 254 Blank43 Respondent for the 24-hour dietary DRPRESP recall interview 10325 1 Sample Person, i.e. self reported 4554 2 Proxy 493 3 Sample Person and Proxy 258 8 Blank but applicable 254 Blank44 Language of 24-hour dietary recall DRPLANG interview 13248 1 English 1895 2 Spanish 133 3 English and Spanish 96 4 Other language 258 8 Blank but applicable 254 BlankNHANES III Examination Data File------------------------------------------------------------------------------------------------------------------------------------------------ Positions Item description Source SAS name Counts and code and notes ------------------------------------------------------------------------45 How does the amount of food consumed DRPQ1 yesterday compare with (your/his/her) usual consumption for that day of the week? In the case of children 6-11 years, the child and a proxy often participated in the interview. Interviews completed with the Sample Person and a proxy respondent were coded as Sample Person/Proxy.1 Sample Person: The Sample Person (Examinee) completed the interview without assistance from another person. This includes situations when a translator was used during the interview to translate information provided solely by the respondent.2 Proxy: Someone else who answered on behalf of the Sample Person. This includes parents, guardians, siblings over 11 years old, caretakers, baby-sitters, and persons responsible for planning or preparing foods eaten by the Sample Person.3 Sample Person/Proxy: The Sample Person and proxy both contributed information during the interview.DRPRDAY Recall DayDRPRDAY corresponds to the day of thw week for the 24-hour period (midnight to midnight) in which the Sample Person consumed the foods and beverages listed in their 24-hour recall. This is the day prior to their examination.DRPNKC Percent of kilocalories from carbohydrateTotal carbohydrates include sugars and complex carbohydrates. Carbohydrate values for foods were derived by difference which may overestimate the carbohydrate content of the food and the percentage of food energy derived from carbohydrate.DRPQ2A Quantity of plain drinking waterThe quantity of plain drinking water was reported either in total fluid ounces per day or by specifying the number of glasses of water and the volume per glass using household glasses. All responses were converted to fluid ounces. If the respondent answered 'none', meaning that no plain drinking water is usually consumed, the quantity was reported as xxx.xx fluid ounces. The volume of plain drinking water is in addition to water used in prepared foods and reconstituted beverages reported in the 24-hr recall.DRPQ5-DRPQ11 Food sufficiency questionsSimilar questions about food sufficiency were also asked of a family respondent in the Family Questionnaire (see HFF4-8). It is recommended that the food sufficiency questions from the Dietary Recall (DRPQ5 - DRPQ11) be analyzed independently from the food sufficiency questions from the Family Questionnaire (HFF4-8).
