Scientific Data DocumentationAlcohol Supplement, 1988DSN: CC37.NHIS88.ALCOHOL ABSTRACT Each year, the National Center for Health Statistics (NCHS) conducts the National Health Interview Survey (NHIS). The NHIS is a personal interview household survey which uses a nationwide sample of the civilian, noninstitutionalized population of the United States. It includes a basic health and demographic questionnaire, that remains virtually unchanged from year to year, and one or more special health topics questionnaires that vary annually. In 1988, the NHIS included a special health topic questionnaire about alcohol consumption patterns and related subjects. It was asked of one randomly selected sample person 18 years of age or older in each interviewed family. The overall response rate for the Alcohol survey was about 87 percent, yielding 43,809 interviews. METHODOLOGY The tape contains data from the NHIS basic health and demographic questionnaire in the first section, and data from the alcohol questionnaire beginning in location 336. The location and arrangement of specific alcohol data fields is different from the order in which the corresponding questions appear on the questionnaire. Generally, the alcohol-related fields are laid out in descending order of the number of types of drinkers asked those questions. In the questionnaire, different types of drinkers were asked different sets of questions, but many of the questions were identical or similar. (See the next paragraph for definitions of the types of drinker.) In the interests of economy of space and ease of use, the originally separate fields for similar questions asked of different types of drinkers have been condensed. To aid the user in finding data locations, this document includes a Table of Contents and an index of questions and their tape locations. The answers given by the various categories of drinkers can be distinguished by using the drinking status recode in location 339. The answers given by the different categories of current drinkers can be distinguished by using the recode in location 563. The respondent was asked to assess the drinking status of each other adult in the family. These individuals were identified by person number. Some information from the NHIS basic health and demographic questionnaire about each of these family members was added to the data tape: sex, age, relationship to the household reference person (the only relationship code available), and education. This data, for up to six persons, is found in locations 447 to 494. Definition of drinking status The categories of drinking status that are found in location 339 are based on answers to the first three questions (locations 336 to 338). "Abstainers" reported that they had never had 12 drinks in their entire life. "Lifetime infrequent drinkers" reported that they had never had 12 drinks in any one year. "Former drinkers" had had 12 drinks in a single year but not in the year prior to the interview. "Current drinkers" reported that they had had 12 or more drinks in the year prior to the interview. In the case of missing data in the responses to any of these initial questions, if the interviewer continued with one of the four sections as though it were appropriate, the missing responses were imputed and the case appropriately classified. If the correct original skip pattern could not be determined and the case could not be classified with reasonable certainty, it was eliminated. Both of these circumstances involved a very small number of cases. Limitations in the logical editing of the data It is generally acknowledged that reporting on the details of an individual's drinking patterns may be inconsistent. It was decided that some apparent inconsistencies in data would be allowed, particularly if they were not immediately adjacent questions. Some inconsistencies were not allowed, however; for example, if the number of days the respondent reported having 9 or more drinks exceeded the number of days when they had 5 or more drinks, the number of days with 5 or more drinks was increased to be consistent with the higher figure. Fields which contain counts of events, for example, the number of drinks per day, were not edited for reasonableness so that each analyst could make his/her own assumptions about acceptable limits; cases with out-of-range values can be eliminated, or values can be truncated or recoded according to the judgement of the analyst. In most cases, the maximum number was limited only by the number of digits and the use of codes "8" and "9" or "98" and "99" etc. as special codes. Weights Except for exploratory runs for preliminary data analysis, data from the NHIS should always be weighted . This data tape contains two weight fields: the final annual basic weight in loc. 201-206 and the two week weight in location 207-215. The final annual basic weight is the appropriate weight for all person-level variables, such as occupation, the number of 12-month doctor visits, and self-assessed drinking status. When using variables collected for two week periods, either from the basic health and demographic questionnaire or from the alcohol questionaire, the value (e.g., the number of doctor visits in the previous two weeks or the amount of beer drunk in two weeks) should be multiplied by the two week weight to calculate the annual estimated number. Calculation of Drinking Level and Average Daily Consumption The last time the NHIS included a detailed survey of alcohol consumption and related variables was 1983. Recodes entitled "Constructed Drinking Level" and the "Average Daily Consumption" of ethanol were included on those tapes. These were only two of many ways of classifying people by consumption. These variables are not included on the current tape but specifications for creating them are available upon request from the Illness and Disability Statistics Branch of NCHS.
