DATA PURCHASE AND USE AGREEMENT

 1987 National Medical Expenditure Survey Data

 Individual identifiers have been removed from the micro-data
 tapes available from the Agency for Health Care Policy and
 Research through NTIS.  Nevertheless, under sections 308(d)
 and 903(c) of the Public Health Service Act (42 U.S.C. 242m
 and 42 U.S.C. 299 a-1), data collected by the Agency for
 Health Care Policy and Research may not be used for any purpose
 other than the purpose for which it was supplied.  The information
 on the micro-data tapes available for purchase was supplied to the
 Agency for statistical summaries and health services research.  It
 is necessary, therefore, that the individual ordering such micro-
 data tapes sign the following assurance:

 The user gives assurance that individual elementary unit
 data on the micro-data tapes being ordered will be used solely for
 statistical summaries and health services research.
BACKGROUND

     This documentation describes one in a series of public use
     tapes issued by the Agency for Health Care Policy and Research
     with data from the National Medical Expenditure Survey.  This
     survey provides extensive information on health expenditures by
     or on behalf of families and individuals, the financing of these
     expenditures, and each person's use of services.  The National
     Medical Expenditure Survey (NMES) is a research project of the
     Center for General Health Services Intramural Research, Agency
     for Health Care Policy and Research.

          Since the 1970s the intramural research program has given
     particular emphasis to studies of the use and financing of health
     services.  The first series of studies (NMES-1) employed data
     collected in the 1977 National Medical Care Expenditure Survey.
     NMES-1 produced information on a broad range of issues such as
     the number and characteristics of the uninsured and the
     underinsured, the tax implications of excluding employer-paid
     premiums for health insurance from employee income, and the
     differences among socioeconomic and demographic groups with
     respect to the use of health services.

          A new series of studies (NMES-2) was initiated in the 1980s.
     These studies also involve a major data collection effort - the
     1987 National Medical Expenditure Survey.  Like its  predecessor,
     NMES-2 provides information about the non-institutionalized
     population.  In addition and in contrast to the earlier studies,
     NMES-2 also provides extensive information on the population
     residing in or admitted to nursing homes and facilities for the
     mentally retarded.

          The Household Component of NMES-2 is based on a national
     probability sample of the civilian, noninstitutionalized
     population living in the community.  The sample is designed to
     provide a larger representation of population groups of special
     policy interest to the Federal Government than would have been
     obtained from a random sample. These groups include poor and low
     income families, the elderly, the functionally impaired, and
     black and Hispanic minorities.  A Survey of American Indians and
     Alaska Natives includes a separate sample of American Indians and
     Alaska Natives living on or near Federal reservations and
     eligible to receive care provided or supported by the Indian
     Health Service. The Institutional Population Component includes a
     sample of persons residing in or admitted to nursing and personal
     care homes and facilities for the mentally retarded during 1987.
     A separate Medicare Records Component provides claims data on all
     Medicare beneficiaries included in the household and
     institutional samples.

          Together, the major components of NMES-2 contain information
     to make national estimates of health status, use of health
     services, insurance coverage, expenditures, and sources of
     payment for the civilian population of the United States during
     the period from January 1 to December 31, 1987.  Oversampling of
     population groups of special interest makes possible in-depth
     studies of these groups.  The database can also be used to assess
     the implications of recent or proposed changes in public or
     private health care benefits, methods of financing both health
     care and insurance coverage, various public and private subsidies
     for health care, and  employee compensation arrangements.

 Household Survey

          Each family in the Household Survey was interviewed four
     times over a period of 16 months to obtain information about the
     family's  health and health care during calendar year 1987.
     Baseline data on household composition, employment, and insurance
     were updated at each interview, and information was obtained on
     illnesses, use of health services, and health expenditures for
     each family member.  A fifth round of interviews was conducted in
     the spring of 1988 to obtain information on the tax filing and
     medical deductions of each household.  A long term care
     supplement was administered during the first and fourth rounds of
     interviewing to permit estimates of persons with functional
     disabilities and the use of formal services or long term care
     provided by family or friends.

          In order to verify and supplement the information provided
     by household respondents, the Household Component of NMES-2
     included two additional surveys. The Medical Provider Survey
     obtained information from the physicians, hospitals, outpatient
     clinics, emergency rooms, and home health agencies used by the
     household sample during 1987.  The Health Insurance Plan Survey
     obtained information on the private insurance of persons in the
     household sample, including premiums paid by all sources and the
     provisions of their coverage.

 Survey of American Indians and Alaska Natives (SAIAN)

          This component was conducted with the same data collection
     instruments and interview procedures as the Household Component
     and covered the same reference period, calendar year 1987.  SAIAN
     also included follow-up surveys to medical providers and health
     insurers.  Consequently, the data can be used to compare American
     Indians and Alaska Natives eligible for care from the Indian
     Health Service and the general U.S. population with regard to
     such issues as health status, use of health services, and access
     to  care.  Information was obtained on services provided outside
     the Indian Health Service and on other sources of health care
     financing available for persons eligible for care from the IHS.

 Institutional Population Component

          The Institutional Population Component of NMES-2 included
     persons resident in or admitted to nursing and personal care
     homes and facilities for the mentally retarded at any time in
     calendar year 1987.  This survey provides information on the
     functional status, use of services, and health expenditures of
     the institutionalized population.  The Survey in Institutions
     (SII) collected data from facility administrators and designated
     staff on the characteristics of facilities and charges.  The
     Survey of Next of Kin (SNK) obtained data from the respondent's
     next-of-kin or other knowledgeable persons in the community on
     the financial status, insurance coverage, and personal history of
     the institutionalized person.

 Survey Samples

          All survey components were designed to provide statistically
     unbiased estimates that are representative of the civilian
     population of the United States in 1987.  The Household Survey
     sample is a stratified multistage area probability design with a
     total sample of roughly 35,000 individuals, in 14,000 households,
     who completed all rounds of data collection.  Oversampling of the
     population subgroups of special policy interest was based on a
     separate screening interview conducted in the fall of 1986 with a
     sample of 36,000 addresses.

          The Survey of American Indians and Alaska Natives adopted a
     multistage area probability sample design using an IHS-
     constructed frame of counties with individuals eligible for
     services provided or supported by the Indian Health Service and
     living on or near Federally recognized reservations or in Alaska.
     An initial screening interview was completed in approximately
     13,700 dwelling units to identify the eligible sample.  The
     screening yielded approximately 1,950 households responding for
     the full year and approximately 6,500 SAIAN persons responded for
     their entire period of eligibility in 1987.

          The institutional population sample was based on a three
     stage probability design. The first two stages were used to
     select facilities; the final stage sampled facility residents
     present on January 1, 1987.  These facilities were also used to
     obtain a sample of admissions between January 1, 1987, and
     December 31, 1987.  Based on sampling specifications the
     Institutional Population Component includes 1,500 facilities, 800
     nursing homes and 700 facilities for the mentally retarded.
     There is a total of approximately 10,100 persons in the sample
     including both residents and new admissions.  This includes 5,700
     persons in nursing homes and 4,400 persons in facilities for the
     mentally retarded.  The sample frame for facilities in the
     Institutional Population Component was derived from the 1986
     Inventory of Long-Term Care Places.

         Taken in conjunction, the NMES-2 surveys yield compre-
     hensive, population-based information that will support studies
     of most population groups of policy interest, including those
     presently outside the scope of various public and private
     financing mechanisms.  In contrast to information obtained from
     program or provider statistics, NMES-2 data can be used to
     analyze all public and private sources of coverage for health
     care services and out-of-pocket payments by individuals and
     families.

          The Agency for Health Care Policy and Research sponsored the
     NMES-2 data collection activities.  A substantial part of the
     support for the Survey of American Indians and Alaska Natives was
     provided by the Indian Health Service.  The Health Care Financing
     Administration, the National Center for Health Statistics, and
     the Office of the Assistant Secretary for Planning and Evaluation
     provided extensive technical assistance during the development of
     the survey design and instruments.  Interviews were conducted by
     the primary contractor, Westat, Inc., Rockville, Maryland and by
     NORC, University of Chicago; the Council of Energy Resource
     Tribes, Denver, Colorado; and Stephen R. Braund and Associates,
     Anchorage Alaska.  Data processing during the analysis stage of
     the project is being provided by Social and Scientific Systems,
     Inc., Bethesda, Maryland.

          The data were collected under the authorities of the Public
     Health Service Act and are being edited and published in
     accordance with the confidentiality provisions of that Act and
     the Privacy Act.

TECHNICAL AND PROGRAMMING INFORMATION

 General Information

         This documentation describes the first public use tape from
     the Survey of American Indians and Alaska Natives (SAIAN), a
     component of the 1987 National Medical Expenditure Survey (NMES).
     The sample population consists of civilian noninstitutionalized
     American Indian and Alaska Natives eligible for the Indian Health
     Service (IHS) and living on or near federally recognized
     reservations or in Alaska in 1987.  The data file provides
     person-level demographic information, including eligibility
     status, and employment and health insurance data for the round 1
     SAIAN population.

         The data on this tape are being released as an EBCDIC file
     only.  The tape also includes an EBCDIC file containing
     programming statements required to create a SAS data set and a
     SAS format library.

         The following documentation offers a brief overview of the
     the type and level of data provided, the content and structure of
     the data file and the codebook, and programming information.  It
     contains the following sections:

         The data on this file are being released prior to final
     cleaning and editing in order to provide access to information
     from the SAIAN.

         The structure of the SAIAN data is comparable to that
     obtained in the NMES Household Survey with regard to
     questionnaire content and data collection procedures.  The
     present public use file was constructed to permit concatenation
     of NMES public use tape 3, which contains demographic, employment
     and insurance data for round 1 of the Household Survey, with the
     corresponding SAIAN variables.

         More detailed information on NMES SAIAN data elements, data
     collection procedures and variance estimation programs are in
     Attachments 1 and 2, which are provided as hard-copy attachments
     to the data tape.  Attachment 3 contains a list of previously
     released NMES public use tapes.  Attachment 4 contains the
     instruments used to collect the information in the data file.

 Eligibility

         The eligible SAIAN population was identified during the
     screener and during round 1 of data collection.  In areas in
     which the population expected to be eligible for SAIAN was
     relatively small, screening began in January 1987.  For high-
     density SAIAN areas, the screener was conducted concurrently with
     the round 1 interviewing. (See the Sample Design section of this
     documentation for additional details.)

         The SAIAN was designed as a national probability sample of
     dwelling units (DUs; these included houses, apartments, etc.).
     Consequently, eligibility was determined in a two-stage process.
     First, DUs were classified as SAIAN eligible or ineligible. Once
     a DU was considered eligible, all persons living within the DU
     were classified as SAIAN eligible or ineligible. DU eligibility
     was determined based upon the following two criteria:

              (1)  The DU was on or near a federally recognized Indian
              reservation or in an eligible county in Alaska.

              (2)  At least one person in the DU was eligible for
              services provided or supported by the Indian Health
              Service.

     Once DU eligibility was determined, all persons within the
     eligible DU were classified as SAIAN eligible if they met the
     following person-level criteria:

              (1) Eligibility for IHS services,

              (2) civilian status, and

              (3) noninstitutional status.

          IHS eligibility was self-reported.  Persons were classified
     as SAIAN eligible if they gave a positive answer to either of the
     following questions (SA28, SA29): "Is (person) eligible to
     receive health services provided by or paid for by the Indian
     Health Service (IHS)?";  "Is (person) eligible to receive health
     services provided by or paid for by a  tribe/inter-tribal
     council/Alaska Regional Corporation (ARC) ?"

         The data file on this tape contains information on 7,071
     persons, that is, all persons for whom any data were obtained in
     an eligible DU.  A total of 6,557 persons who met both DU and
     person-level eligibility criteria are included as key persons for
     round 1 SAIAN.  Using the weight PESAMPWT, they can be used to
     make person-level estimates for the variables on this file.  The
     remaining 514 persons were not IHS-eligible but lived in a family
     with at least one IHS-eligible person, or were institutionalized
     or on active military duty during round 1.  Data for these
     persons can only be used for household level estimates.

 Data File Contents and Codebook Structure

         This public use data set contains one record for each person
     who was identified and responded in an eligible dwelling unit in
     round 1 of the SAIAN, for a total of 7,071 person-level records.
     Each record includes identifiers, eligibility status indicator,
     demographic and family characteristics, employment and health
     insurance status and sampling weight and variance estimation
     variables.

         The reference period for the data contained on this file
     extends from January 1, 1987 or the beginning of round 1
     eligibility to the date of the round 1 interview or the date when
     the person ceased to be eligible for NMES as a member of the
     SAIAN civilian noninstitutionalized population (i.e. date of
     death or institutionalization).  Both the date of the Round 1
     interview and the NMES eligibility begin dates and end dates are
     included on the file.

         Specific data items contained are detailed below.

 Eligibility/Keyness

         A character variable, KEYCODE, was constructed which
     classifies all persons responding in round 1 within an eligible
     DU as to IHS eligibility.   The variable has the following
     values:

         Key Persons (K) -- Civilian noninstitutionalized persons
         eligible for IHS who responded to the round 1 interview.
         Also considered key persons are women pregnant with a child
         eligible for services provided by the IHS.

         Out of Scope (OS) -- Persons eligible for IHS services, but
         on active military duty or institutionalized for the entire
         round 1 reference period.

         Not Eligible (NE) -- Persons not eligible to receive IHS or
         tribal/ARC services.

 Record Identifiers

         The unique record identifier on the file is the variable PIDX
     (ODUX + PN) where:

        ODUX =  the original dwelling unit, i.e., the dwelling unit
                 in which the person lived when he or she entered the
                 survey, and

         PN   =  person number, which is a 3-digit number assigned to
                 each person residing in an original dwelling unit.

 Population Characteristics

  Demographics

         Demographic characteristics were obtained in either the SAIAN
     screener or round 1 interview.  Items not subject to change were
     collected during the screening interview (sex, date of birth,
     race, ethnicity, and prior military service); they were not asked
     again for persons who remained in dwelling units selected for the
     round 1 interview.  Items such as marital status, family
     relationships, current military service, and educational status
     were updated during the round 1 interview.

  Family Relationships

         As in the NMES Household Survey, three household or family
     definitions were employed:

              (1)  Original Dwelling Unit (ODUX), corresponds to a
              sampled address.  All persons living together at this
              address, plus any unmarried college student living away
              from home but whose parents lived at the sampled
              address, were eligible for the round 1 interview and are
              identified by their common value of ODUX.

              (2)  Reporting Unit (RU) corresponds to the group
              of persons covered by a single interview and was defined
              as all persons related by blood, marriage, adoption or
              foster status living together at the time of the
              interview.  For example, two unrelated individuals
              living together at a sampled address comprise one
              Original Dwelling Unit and two RUs.  College students
              living away from home were interviewed separately and
              were assigned to a separate RU, as were any persons who
              requested a confidential interview separate from the
              rest of the family (no cases in round 1 of SAIAN).

              (3)  Family (FAMILIDX), corresponds to all persons
              related by blood, marriage, adoption or foster status
              who were living together at the time of the round 1
              interview plus college students and persons requesting
              confidential interviews who had been assigned to
              separate RUs for the original data collection.

         The family reference person was identified during the
     enumeration as the person who owns or rents the house or
     apartment.  Immediate family members can be determined through
     appropriate use of the variables indicating each person's spouse,
     mother, father and relation to the reference person.

 Employment

         Employment data were obtained for all persons 16 or older at
     the time of interview.  Information about employment status and
     different types of jobs held by sample persons was collected.
     Information was obtained for the last job, if any, held by adults
     who were not employed during the reference period.  All jobs held
     during the reference period are also described, whether the most
     recent main job, a previous job, a second job held at the same
     time as the most recent main job, or any other job during the
     reference period.  For persons who changed jobs during the
     reference period, therefore, there are data for at least two
     types of jobs.  Depending on job type, information is provided on
     hours, pay, union membership, establishment size, sick leave and
     medical leave, self-employment, industry or occupation, job
     descriptor, and dates of employment.

 Health Insurance

         Health insurance coverage under Medicare and CHAMPUS/CHAMPVA
     was determined as of the interview date.  For coverage likely to
     change over time--Medicaid, other public medical assistance (for
     example, state and local programs not subsidized by the Federal
     government), and private insurance--the interview ascertained
     coverage at any time during the reference period.  Respondents
     were specifically told to exclude from private health insurance
     any plan that pays only for accidents, specific dread diseases,
     or provides extra cash during a hospitalization.  Coverage
     through the Indian Health Service is not included among these
     types of public or private coverage.  It is identified on the
     file through variables GETIHS and GETTRIBE (SA28 and SA29).

         If a reporting unit reported any private health insurance as
     defined, then all of the RU's plans were enumerated.  The source
     of each plan (whether an employer or business, union or other
     group, or insurance company) was specifically identified, as was
     the policyholder.  In some instances, as for children insured by
     a divorced parent, the policyholder was not a reporting unit
     member.  Finally, the interviewer ascertained which members of
     the reporting unit were covered by each plan.  The indicators of
     persons with private insurance from one or more sources were
     constructed from any plan reported as covering each person.  The
     coverage reported by students living away from home was related
     to the plans reported elsewhere for the family during this
     process.  Because the policyholder for each plan was also
     identified, indicators were constructed to identify policyholders
     of private insurance generally and from different sources.

         As these variables were constructed from items in the
     questionnaires that are not provided on the tape in their
     entirety, the extent of missing information cannot be determined
     by the user from the file itself.  Approximately 10 reporting
     units were missing information about whether or not they had a
     private health insurance plan.  Among reporting units with
     private insurance, valid responses with respect to the coverage
     of each RU member under each plan were obtained for 92 percent of
     the roughly 2,800 combinations of RU members and plans.  Coverage
     was assigned in the remaining cases from family relationships,
     the other family members covered by the plan, and other
     information, such as Medicare supplementary coverage.

 Codebook Structure

         The codebook provided for this data file provides complete
     variable listings in alphabetical order and by file position for
     cross-reference.  Most variable descriptors in the codebook are
     abbreviated versions of SAIAN questionnaire items, preceded by
     indicators of item or box number.  For constructed variables and
     those which correspond to more than one questionnaire item, the
     variable descriptor does not include questionnaire item numbers.
     A full set of the relevant sections from corresponding screener
     and round 1 questionnaires is provided as hard copy in Attachment
     4 to this public use tape to permit a full understanding of the
     content and wording of each item, the structure of questionnaire
     sections, and skip patterns.

         The codebook provides unweighted and weighted frequencies for
     all variables on the file; all variables are weighted using the
     person level weight PESAMPWT contained on the file.

         Records from this file can be linked to future public use
     data sets from the SAIAN by using the person identifier PIDX.

 Omissions

         Several variables and response categories have been excluded
     from this data file for reasons of confidentiality, such as
     identification of tribe (SA27).  The response to Questions SA47
     and SA48, which asked about coverage by selected HMO plans, were
     omitted but will be released with future NMES public use tapes.
     Other omitted variables are box items for use by the interviewer
     in following skip patterns, interview times and interviewer
     names, and similar items.  Other data omitted include the
     responses to questions SA56 - SA59, which were only asked of
     ineligible households.

 Edits and Imputations

  Editing

         Editing of data was mainly undertaken to correct obvious
     errors such as logical inconsistencies (e.g., for assignment of
     veteran status, all persons under age 17 were assigned to "no
     military service").  Deviations from skip patterns were corrected
     only for edited variables.  For the few variables with extensive
     edits, both edited and unedited versions are provided, together
     with relevant information in the codebook notes.  All edited
     variables, including those with imputed values, are denoted by
     the suffix "ED" in the codebook following the questionnaire item
     number in the descriptor label.  For edited variables only, the
     skip patterns in the employment section of the core questionnaire
     are driven by the responses to questions about the person's age
     and employment status.

  Imputation

         Missing values for hourly wages (HOURWGX2) and weekly hours
     of work (HRSPRWX2) were imputed using the hot deck procedure,
     which imputes data from individuals with complete information to
     individuals with missing data but with similar characteristics.
     Imputation classification variables were: age, gender, education,
     industry, occupation, geographical location, presence of young
     children, and whether the person was self-employed or worked for
     someone else.  These were used to form groups of donors with
     known data on hourly wage or weekly hours of work and identical
     groups of recipients with missing data.  Within such groups, data
     were assigned from donors to recipients, taking into account the
     weights associated with each person in the complex survey design.

 Variable Naming and Codebook Conventions

         The codebook contains variable information and unweighted and
     weighted frequency distributions for a total of 7,071 records.
     The codebook describes an EBCDIC data set and provides the
     following programming identifiers for each variable:

         In general, variable names reflect the content of the
     variable, with an 8 character limitation.  For edited versions of
     original variables, the edited variable name is identical to the
     original variable with an "X" appended (and truncated when
     necessary to comply with the 8 character limitation).  For
     variables corresponding to specific questionnaire items, the
     question number is included in the variable label.

         The following reserved code values are used:

 Sample Design and Response Rates

         The NMES Survey of American Indians and Alaska Natives was
     designed to produce statistically unbiased estimates that are
     representative of the civilian noninstitutionalized population
     living on or near reservations and eligible for services provided
     or supported by the Indian Health Service (IHS).  The sample can
     be characterized as a stratified multi-stage area probability
     design which was drawn using an IHS constructed frame of people
     eligible for IHS provided or supported services and living on or
     near federally recognized reservations.  The SAIAN sampling frame
     initially consisted of 482 counties of the United States served
     by IHS, with an estimated population of 1,013,000 projected by
     IHS for 1987.  For cost efficiency the frame was truncated to
     exclude counties with less than 400 Indians or Alaska Natives,
     and 24 counties were paired with larger neighboring counties to
     form 274 PSUs available for sample selection.  The truncated
     frame included 97.2 percent of the population of interest.

         Within each sampled PSU, SAIAN area segments were created.
     Segments were defined as 1980 census enumeration districts or
     individual blocks or block combinations.  For cost efficiency,
     the SAIAN sample was further restricted by excluding segments
     with less than 0.5 percent of the population of American Indians
     and Alaska Natives.  Approximately 95 percent of the SAIAN
     population remained eligible for sample selection when the effect
     of PSU and segment frame truncation were considered jointly.

         In early 1987, an initial screening interview was completed
     in approximately 13,700 dwelling units to facilitate
     identification of the eligible sample.  Data were obtained for
     about 87.5 percent of eligible sampled reporting units in the
     first round of data collection, consisting of 1990 reporting
     units and 6,557 civilian noninstitutionalized persons who
     responded and were eligible for IHS.  For a detailed description
     of the survey design and of sampling, estimation, and adjustment
     methods, including weighting for nonresponse, see Cohen,
     DiGaetano, and Waksberg  (Sample Design of the NMES Survey of
     American Indians and Alaska Natives, Proceedings of the American
     Statistical Association, Survey Research Methods Section, 1988).

 Sampling Weights

         The application of sampling weights is essential to the
     derivation of person- and family-level estimates of the SAIAN
     civilian noninstitutionalized population living on or near
     federally recognized reservations and eligible for Indian Health
     Services in 1987.  To facilitate the derivation of national
     population estimates from the SAIAN data and permit comparison
     with the NMES household survey data, two sampling weights are
     provided on this file:

         (1) PESAMPWT -- A weight for persons who were SAIAN eligible
         and responded to the first round of data collection.   This
         weight should be used when making estimates about persons.
         There are 6,557 persons with positive PESAMPWT weights.  The
         sum of weights for persons with positive PESAMPWT weights is
         884,909.

         (2) RUSAMPWT -- A weight for reporting units (RUs) that were
         SAIAN eligible and had at least one person responding to the
         first round of data collection.  This weight should be used
         when making estimates at the RU level.  There are 1,990 RUs
         with positive RUSAMPWT weights.  The sum of the weights for
         RUs with positive RUSAMPWT weights is 298,640.  To make
         family estimates i.e., estimates at the level of FAMILIDX, it
         will be necessary to link students to their actual RU of
         origin.  This can be done using the variables RUCLASS and
         FAMILIDX.

     The person level weight was adjusted for nonresponse at the
     dwelling unit level, at the reporting unit level, and at the
     person level.  The RU level weight was adjusted for nonresponse
     at the dwelling unit level, and at the reporting unit level.

 Variance Estimation

         The complex nature of the NMES SAIAN design must be taken
     into account for hypothesis testing and for the construction of
     confidence intervals.  To obtain variance estimates of sample
     statistics by means of standard statistical programs that use the
     Taylor series method of variance estimation, variables must be
     used that denote the stratum and the primary sampling unit (PSU)
     within a given stratum.  The variables STRATUMX and SPSU are
     these variables, respectively.  A list of available variance
     estimation programs that account for the complex survey design is
     provided in Attachment 2.

 Programming Information

         These files are contained on a standard label, 9 track 6250
     bpi tape.  The specifications for each file on the tape are as
     follows:

         File 1 was created using the SAS (Statistical Analysis
     System, version 5.18) computer software and converted to EBCDIC
     format.  File 2 contains the technical documentation stored as an
     Operating System (OS) EBCDIC file containing ASA carriage control
     characters in the first byte in each record, which will direct
     the line printer to skip lines, begin a new page, etc.  This
     technical documentation can be copied to disk and retrieved on-
     line to view and/or modify using a text editor program such as
     WYLBUR.  File 3 is an EBCDIC file containing the following
     additional documentation for SAS users:  INPUT statement to
     create the SAS file, including a label statement; SAS statements

 NMES SAIAN Round 1 Person-Level File Codebook Notes

     The following explanatory notes are arranged in alphabetical order
     of variable name.  Question references are to the SAIAN household
     questionnaire.

     Variable                                 Note

     CCRELCOX          CCRELCOX describes a person's relationship to the
                       reporting unit reference person.  A reporting unit
                       differs from a family in that family members who
                       are students living away from home or who requested
                       confidential interviews are in their own reporting
                       units.


     CCSTATUS          A constructed variable identifying eligibility
                       status as of the time of interview.  It can be used
                       to identify why a person failed to provide data for
                       the entire reference period (e.g., died) or was
                       classified as out of scope.  (e.g., was on active
                       military duty).

                       Other status codes used during the survey period
                       were not applicable for round 1 of data collection.


     CONDIND(1-3)      Condense the 3-digit industry and occupation codes
     CONDOCC(1-3)      described in the note for JOBIND(1-3) and JOBOCC(1-
                       3) into the major industry and occupational groups
                       defined by the Bureau of the Census.  (See Bureau
                       of the Census, 1980 Census of Population:
                       Alphabetic Index of Industries and Occupations.)


     DESCENT           A constructed person level variable reflecting
                       descent for all people who reported themselves as
                       eligible for IHS or Alaska Regional Corporation
                       services. It takes on the following values:

                       1     Self-reported American Indian or Alaska Native
                             (see SMPRABKG and SMPRABKX).

                       2     Descendants of American Indians and Alaska
                             Natives as determined from their family
                             relationships;

                       3     Member or eligible to be member of a tribe

                       4     Other races

     EMPLOYDX          An edited variable indicating that PID was employed
                       during the Round 1 reference period as indicated by
                       a positive response to WDOWORKX (question B1) or
                       WHAVEJBX (question B2).


     FAMILIDX          A person-level variable which identifies the family
                       of all individuals related by blood, marriage,
                       adoption or foster status and living in an eligible
                       dwelling unit.  Includes students living away from
                       home and individuals requesting confidential
                       interviews who were interviewed in reporting units
                       separate from the regular reporting unit as (see
                       HOMEIDX and RUCLASS).


     FAMREFPN          FAMREFPN indicates the person number (PN) of the
     FAMREL            family reference person for every family member.
                       FAMREL describes the relationship of each family
                       member to the family reference person  (see
                       FAMILIDX).


     GETIHS            This variable corresponds to question SA28 from the
                       SAIAN enumeration booklet.  It identifies DU
                       members eligible for services provided or supported
                       by the Indian Health Services. Values are as
                       follows:

                             -8  Don't Know

                             -9  Not Ascertained

                             -1  Inapplicable

                             1  Yes, eligible

                             2  No, not eligible

                             3  Mother not eligible but pregnant with
                            eligible child.

                       Category 3 was not listed on the questionnaire and
                       was constructed from data items collected elsewhere
                       in the SAIAN NMES.


     HOMEIDX           The identification number for each separate
                       reporting unit (see RUCLASS).  It consists of a
                       five digit ODUX, a check digit, a RU indicator, and
                       the round number.

     HOURWGE2          Constructed variables indicating hourly
     HOURWGE3          wage rates for persons not paid on an hourly basis
     HOURWGX2          by using information regarding payment amounts, the
                       time period covered by the payment, and the hours
                       and days usually worked per week.  Wage rates for
                       HOURWGE2 and HOURWGE3 have been edited for outliers
                       but no missing values have been imputed.  The
                       variable HOURWGX2 includes imputation for missing
                       values and editing for outliers.


     HPRIV             A constructed variable indicating that PID was
                       named as the policyholder of one of the RU plans
                       (Questions T20/27/34/41).


     HPRIVEG           Constructed variables based on plans from employ-
     HPRIVNG           ers, unions, and other groups, or directly from in-
     HPRIVOG           surance companies (nongroup coverage) as identified
     HPRIVU            from questions T17 and T18 (or the repetition of
                       these questions for RUs with more than one plan).
                       PIDs are indicated as holding a policy from one of
                       these sources if named as the policyholder for a
                       plan from that source according to T20/27/34/41
                       (see HPRIV).

     JOBIND(1-3)       JOBIND(1-3) and JOBOCC(1-3) represent, respective-
     JOBOCC(1-3)       ly, the 3-digit industry and occupation codes
                       defined by the Bureau of the Census for the 1980
                       Census for the jobs described in S16JOB1, S21JOB2,
                       S43JOB3.  This coding system is consistent with the
                       Standard Industrial  Classification (SIC) and
                       Standard Occupational Coding (SOC) systems,
                       although the numeric values or codes assigned to
                       each category differ.


     KEYCODE           This is a constructed character variable indicating
                       person level eligibility for round 1 SAIAN.  It
                       takes the following values:

                       K (Key) -- All DU members eligible for Indian
                       Health Service or tribal Alaska Regional
                       Corporation services in Round 1.  Women pregnant
                       with a child eligible for IHS services are also
                       considered Key.

                       OS (Out of Scope) -- A person eligible for IHS
                       services or tribal/Alaska Regional Cooperation
                       services, but on active military duty or
                       institutionalized for the entire round 1 reference
                       period.

                       NE (Not eligible) -- Not eligible to receive IHS or
                       tribal/Alaska Regional Corporation service.


     PESAMPWT          The SAIAN person level weight adjusted for
                       nonresponse.  Must be used to produce unbiased
                       person level estimates of the civilian
                       noninstitutionalized SAIAN population.  For details
                       on sampling weights and variance estimation, see
                       the Technical and Programming section on this file
                       and the hard copy documentation accompanying this
                       tape.


     PRIV              A constructed variable indicating the private
                       insurance coverage of persons as determined from RU
                       coverage (question: T16) and from PID coverage
                       under the family's plans (Question: T47/52/57/62).


     PRIVEG            Constructed variables, where plans from employers,
     PRIVNG            unions, and other groups or directly from
     PRIVOG            insurance companies (nongroup coverage) were
     PRIVU             identified from question T17 and question T18 (or
                       the repetition of these questions for RUs with more
                       than one plan).  PIDs are indicated as having
                       coverage from one of these sources if covered by a
                       plan from that source according to questions
                       T47/52/57/62.


     PRIVOUT           A constructed variable indicating person coverage
                       by some "other group" according to questions T17
                       and T18 (or the repetition of these questions for
                       families with more than one plan), but through a
                       policyholder outside the reporting unit.  For
                       example, this applies when coverage of children
                       insured by a divorced spouse was reported as "other
                       group" coverage.


     RUCLASS           RUCLASS identifies reporting units as regular,
                       requesting confidential interviews, or students
                       living away from home (see FAMILIDX).  There were
                       no cases requesting confidential interviews in
                       round 1.


     RUSAMPWT          The SAIAN reporting unit weight.  Used to produce
                       unbiased RU level estimates.


     S16JOB1           Indicators provided to identify data about several
     S21JOB2           types of jobs held by PID.  S16JOB1 indicates a
     S43JOB3           job prior to January 1, 1987, either a job from
     S61JOB4           which the worker retired or the last job held if
     S66JOB5           not employed during the reference period.  S21JOB2
                       indicates the most recent main job held during the
                       reference period, typically the main job at the
                       time of interview.  S43JOB3 indicates a job held on
                       January 1, 1987 that preceded the most recent main
                       job during the reference period (S21JOB2).  S61JOB4
                       indicates a second job held at the same time as the
                       most recent main job (S21JOB2).  S66JOB5 indicates
                       any other job held during the reference period not
                       encompassed by S21JOB2, S43JOB3, and S61JOB4.


     SMPDISAX          An edited version of SMPDISAB, which was modified
                       to be consistent with the constructed variable VET.
                       It indicates veterans with a service-related
                       disability.


     SMPRABKG          Person-level variable including self-reported
     SMPRABKX          racial background (see also DESCENT).  It takes the
                       following positive values:

                             1     American Indian
                             2     Alaska Native
                             3     Asian or Pacific Islander
                             4     Black
                             5     White
                             6     Other


     SPSU              Variables denoting the stratum and the Primary
     STRATUMX          Sampling Unit (PSU) within a given stratum.  These
                       variables must be used to obtain variance estimates
                       of NMES sample statistics by means of standard
                       statistical programs that use the Taylor series
                       method of variance estimation.

                       For details on sampling weights and variance
                       estimation, see the Technical and Programming
                       section on this file and the hard copy
                       documentation accompanying this tape.


     VET               Constructed variable indicating persons who were
                       veterans at the time of the round 1 interview.
                       Veteran status for this variable is defined in
                       accordance with the definition used by the
                       Department of Veteran's Affairs (persons age 17 or

                       older with prior service in either the Armed Forces
                       or a National Guard/military reserve unit).  Active
                       duty military personnel are not classified as
                       veterans.


     WDOWORKX          Edited variable indicating that PID worked at a job
                       or business (including unpaid work in a family farm
                       or business) at any time during the reference
                       period.


     WHAVEJBX          Edited variable indicating that PID had a job or
                       business even if reported as not working during the
                       reference period.  This variable was only asked if
                       respondent answered "NO" to WDOWORKX (question B1).
